Ne vyzyvaet somneniya vedushchaya rol' immunopatologicheskikh reaktsiy v patogeneze rasseyannogo skleroza (RS). Osnovnym patomorfologicheskim substratom zabolevaniya yavlyayutsya ochagi demielinizatsii v tsentral'noy nervnoy sisteme (TsNS). Odnako morfologicheskim izmeneniyam pri RS podvergaetsya ne tol'ko mielinovaya obolochka nervnykh volokon, no i neposredstvenno aksony. Imenno aksonal'naya degeneratsiya yavlyaetsya prichinoy razvitiya stoykoy patologii v TsNS, privodyashchey k ranney invalidizatsii bol'nykh. Nalichie obshchikh strukturnykh i mediatornykh zven'ev nervnoy i immunnoy sistem obuslavlivaet sochetannye i vzaimozavisimye narusheniya funktsii TsNS i immunnoy sistem v patogeneze zabolevaniya. V svyazi s etim otsenka povrezhdeniya TsNS pri RS provoditsya parallel'no s primeneniem neyroradiologicheskikh i immunologicheskikh metodov.
